Instructions - Affidavit for Duplicate Warrant

An Affidavit for Duplicate Warrant is used when a real check has legitimately not been received by the employee nor has the employee cashed a real check and then tried to ask for a duplicate to get more money. The Affidavit is the form used by the Department of Finance and Administration (DFA) to make sure the duplicate check is not extra money. It is a process that can take up to 8 - 10 days for the replacement.


If you are paid with a real check (warrant) and have not received it at home or at work, whichever location you specified, call your HR representative to let them know it has not come. An Affidavit for Duplicate Warrant will be faxed or mailed to you or you can download it from the AOC/HR website. It will be partially filled out by your HR representative. Fill out the remainder of the form, sign it and have the form notarized. Just to be sure, call your HR representative to make sure the information is exactly correct, or DFA will send it back to you to
correc the information and slow down the process. Make a copy for your records.

DFA must receive the original, not a fax copy, as quickly as you can get it to them. Mail, Fed-EX or hand carry it to your HR representative and they can get it to DFA or directly to the DFA in Santa Fe at the following address: Department of Finance and Administration, General Payroll, 180 Bataan Memorial Building, Santa Fe, NM 87501. Once DFA has received the Affidavit, they will cancel the original check, process the affidavit and send out a duplicate check.


DO NOT CASH IT. Return the check and it's envelope immediately to your HR representative. They will make sure it gets to DFA.
